Change of Major Form (.doc, 32K)
It is strongly encouraged that you seek advising from your current advisor or the intended department chair before changing your major. Any change of major may impact your graduation date. You may also seek information from Career Services regarding your career path, jpreffer@flagler.edu.
 
Once your new major or minor is determined, fill out the attached form and bring to the Office of the Advising and Retention, located in the Ringhaver Student Center.
Field Trip Release Form (.doc, 28K)
Please print the form; you will need to sign it and have it notarized.
Return the completed form to your professor.
Spring 2007-2008 Calendar (.doc, 123K)
Request for Excused Absence (.doc, 33K)
Fill out this form and bring it to the Office of the Assistant Dean of Academic Affairs. Attach any documentation available and see Dean Yvan Kelly.
If he approves this request, the excuse will be sent to all professors of missed classes.
